PARTICIPATION AGREEMENT
My child, ward, or person for whom I am legal guardian, the “Camper,” seeks participation in Alley Play Makers, the “Program.” In consideration for the Camper’s participation in the Program, I agree to the following:

I. I agree that I or another parent or guardian will be available at all times by telephone while the Camper is participating in the Program.

II. I understand that the Camper may be temporarily or permanently suspended from the Program, without refund, if the Camper engages in improper conduct, including, but not limited to:

Disruptive, harassing, or reckless behavior
Defacing, disfiguring, damaging, or destroying public or private property during program activities
The threat or commission of physical violence against any person present during program activities
Theft or attempted theft
Failure to comply with directions of program officials acting in the performance of their duties

III. I understand that during the Program, the Camper may perform one or more of the following tasks:

Participate in exercises and activities that require physical exertion, reading out loud, memorizing texts, memorizing sequences of actions or choreography, writing and drawing.

IV. I agree that I, or another parent or guardian, will be responsible for transporting the Camper to and from the assigned Camper drop-off and pick-up location according to the scheduled program start and end times.

V. I agree that I, or another parent or guardian, will accompany and remain with the Camper if the Camper arrives prior to the scheduled program start time.

VI. I agree that I, or another parent or guardian, will pick up the Camper within 15 minutes of the scheduled program end time.

VII. I agree that if the Camper arrives 10 minutes or more after the scheduled program start time, I will accompany the Camper into the building to check-in with the Camper’s Teaching Artist.

CONSENT AND RELEASE On behalf of the Camper, myself, my heirs, executors, administrators and assigns, I hereby consent to the Camper’s participation in the Program and I release, hold harmless and forever discharge The Alley Theatre (the “Theatre”) and its officers, owners, agents and employees, of and from any and all liability and claims, for damages, expenses, personal injury or death, which may arise in the future, related to, connected with, or growing out of participation in the Program, including, but not limited to, liability and claims arising from the negligence or gross negligence of the parties hereby released. I understand that the Theatre does not provide any hospitalization or medical insurance to cover the Camper for hospital or medical expenses incurred related to participation in the Program and I am solely responsible for the payment of any and all hospital and/or medical bills. I consent to any first aid care that the Theatre provides for the Camper, but I understand that the Theatre is not obligated to provide any such care.

As of November 2022, masking, vaccinations, and social distancing are not required of Alley Theatre audience members or students. This policy will remain in effect while community transmission levels are low. Should community transmission levels rise, Alley Theatre maintains the right to update its policy in accordance with CDC recommendations and state and local advisement. By selecting “I agree,” Conservatory students and parents acknowledge and agree to follow all Alley COVID-19 Safety Protocols at a given time. Should the situation change, Conservatory tuition will not be refunded to families who chose not to follow updated protocols.
